Sample forms

All forms shown as samples to illustrate their completion have been removed from the handbook and placed on the OCC.

Group 1 and group 2

The following two forms are available on IBIS for online completion, they are not available in PDF for submission as hardcopy.

  • C2 - Group 1 special request subject
  • 1/A1AP - Group 1 advance notice of works studied

However, if the online version of either C2 or 1/A1AP cannot be completed online because non-Roman script must be used, print-off a screen shot of the 'form' on IBIS and submit it to IB Cardiff as hardcopy.

All forms that support internal assessment for the current courses in group 1 and group 2 are available in writeable PDF. In other words, these forms can be completed by the coordinator (or by the teacher accessing each form through the OCC) online, saved in a file, and then uploaded with the sample oral work submitted to the IB electronically for the purpose of moderation. No special software is required for this process. Only Adobe Reader (which can be downloaded from the internet at no charge) is required. The forms are:

  • 1/IARF - Group 1/internal assessment record form (for school use only)
  • 2/IARF - Group 2/record form (for school use only)
  • 2/IA - Group 2/internal assessment
  • 2/CLCS - Group 2/classical languages coversheet

Note that forms 2/RFAI, 2/RFB and 2/RFA2 have been integrated into one form (for school use only). Form 2/IACL - Group 2/internal assessment classical languages has been removed from the handbook. It was considered unnecessary because the information is duplicated through form 3/CS.

Group 3: all subjects

Form 3/IA has been removed from the handbook. It was considered unnecessary because the information is duplicated.

Group 4

No changes.

Group 5: mathematics subjects

Form 5/IA has been removed from the handbook. It was considered unnecessary because the information is duplicated.

Group 5: computer science

Form 5/IACS (HL) and 5/IACS (SL) have been integrated to create one form named 5/IACS (Group 5/internal assessment coversheet).

Group 6: theatre

The following forms have been removed from the handbook.

  • 6/IATpresentation - Group 6/internal assessment theatre presentation
  • 6/IATportfolio - Group 6/internal assessment theatre portfolio

They were considered unnecessary because the information is duplicated through forms:

Group 6: film

Form 6/IAF portfolio has been removed.

Theory of knowledge

Forms TK/PDD and TK/PMF have been integrated to create one form named TK/PPM (Theory of Knowledge/planning and presentation marking).

Creativity, Action, Service (CAS)

The form CAS/PCF (CAS/programme completion form) is now electronic on IBIS. There is no hardcopy version available in PDF.

School-based syllabus subjects

Form SBS/CS has been replaced with a new 3/SBSCS and form SSX has been removed because it is no longer necessary.

Form SBS/TREP has been removed. Feedback on the SBS examinations papers (where more than one school has candidates for the subject) can be submitted on the form Teachers' feedback on the examinations papers (G2) available on the OCC.
